How Do I Apply?
If
you feel that you are a candidate for a scooter from the Clarence
Vincent Foundation (CVF), you may apply through one
of our Partnering Organizations.
You
may also be nominated by a third party organization or by
another person. In this case, the nominating party/individual
must visit the Nominate a Scooter
Recipient page and follow the directions shown.
Certificate
of Medical Necessity
A
Certificate of Medical Necessity is required from
all applicants (available from your doctor). This certificate
is used to determine if the scooter is a medical necessity.
Physical requirements range from individuals who cannot walk
other than very short distances to those with weakness of
the upper extremities that prevent them from self-propelling
a manual wheelchair.
Applicants
will be considered based on a set of Recipient
Criteria as outlined by the CVF. The applicant's
physician must certify the criteria and must also provide
any narrative statements that might be necessary to qualify
the request.
CVF
will not provide a scooter if Medicare (or other agency) has
previously covered the rental or purchase of a scooter for
the applicant. In such as case, the applicant's existing scooter
must be returned to the company that supplied it before being
considered for a scooter from CVF. CVF will not consider requests
for applicants who already own a scooter or powered wheelchair.
Note:
The CVF does not accept direct inquiries and relies solely
on the expertise of our Partnering
Organizations for applicant screening and submission.
