Recipient Criteria
A
Certificate of Medical Necessity is required from
the applicant's doctor. This certificate will be used to determine
if the scooter is a medical necessity. Physical requirements
range from individuals who cannot walk other than very short
distances to those with weakness of the upper extremities
that prevent them from self-propelling a manual wheelchair.
Applicants
will be considered based on the following criteria:
- The
applicant's condition is such that without the use of a
wheelchair they would be confined to their home.
- The
applicant's condition is such that a scooter is medically
necessary and they are unable to operate a manual wheelchair.
- They
are capable of safely operating the controls of a scooter.
- They
require the use of a scooter to move around their residence.
- They
have weakness of the lower extremities due to age, a neurological,
muscular, or cardiopulmonary disease or condition.
- Their
condition is such that a scooter will be needed long term
(indefinitely).
They do not presently own a scooter or powered wheelchair.
- They
cannot otherwise afford a scooter.
The
applicant's physician must certify the above information on
the applicant's behalf. Their physician must also provide
any narrative statements that might be necessary to qualify
the claim. This could include a mobility evaluation by a Physical
Therapist or Occupational Therapist or progressive chart notes
and history of treatment from the applicant's patient file.
The
Clarence Vincent Foundation (CVF) will not
provide a scooter if Medicare (or other agency) has previously
covered the rental or purchase of a scooter for the applicant.
In such as case, the applicant's existing scooter must be
returned to the company that supplied it. CVF will not consider
requests for applicants who already own a scooter or powered
wheelchair.
